My legs are always swollen and achy at the end of the day - could it be a sign that something's wrong?

Dr Philippa Kaye answers: It's normal to feel tired after a long day, particularly if you've been on your feet, but more marked swelling and aching in the legs by evening could indicate something else.

The most common reason is oedema - a buildup of fluid under the skin - which tends to collect in the feet and ankles due to gravity.

It can happen simply from standing or sitting still for too long, and is more common in hot weather, during pregnancy, if you're overweight, or as a side effect of certain medicines such as some used for high blood pressure.

Varicose veins are another cause. Here, the tiny valves in the veins stop working properly so blood pools, making veins bulge and twist.

This can cause swelling, heaviness, aching and itching, and sometimes skin colour changes.

More rarely, swelling can be linked to peripheral arterial disease, where fatty deposits build up in blood vessels and reduce blood flow to the legs.

This can cause pain - especially when walking - along with pale or shiny skin and, in severe cases, ulcers. Risk factors include smoking, obesity, diabetes, high cholesterol and high blood pressure.

See your GP, who can check circulation and vein health.

Treatment depends on the cause, but it can help to avoid sitting or standing for long periods, keep active, wear comfortable shoes and elevate your legs when possible.

Sudden swelling or pain in one leg only warrants urgent medical attention as it could indicate a blood clot.

I keep noticing strange flickering twitches under my skin (my eyelid and lately my forearm). Could it be motor neurone disease?

Dr Philippa Kaye answers: The flickering you describe is called fasciculation - tiny muscle twitches under the skin.

Most people experience them at some point, often after strenuous exercise, dehydration or overuse of a muscle, and they usually settle with rest, food and fluids.

In the eyelid this is known as blepharospasm, and can be triggered by tiredness, bright light or staring at screens.

Stress or general illness can also make fasciculations more noticeable.

While fasciculations can occur in motor neurone disease (MND), they're almost always accompanied by other neurological symptoms such as progressive muscle weakness, stiffness, or difficulties with speech and swallowing.

On their own, they're rarely a cause for alarm.

If you're worried, see your GP, who can carry out a neurological examination and refer you if necessary.

Even surgeons look out for the first fart after bowel surgery as a sign the gut has 'woken up'!

A high-fibre diet can increase gas production because certain carbohydrates aren't fully broken down and ferment in the large bowel.

Fizzy drinks, some medications (such as statins and antifungal drugs), constipation, lactose intolerance, coeliac disease and gut infections can also contribute.

However, being unable to hold it in is not normal. The nerves and muscles around the rectum and anus are usually finely tuned, allowing you to distinguish between gas, liquid and solid stool.

Loss of control could indicate a problem with the anal sphincter muscles, the nerves supplying the area, or the bowel itself.

If this is happening, see your GP promptly for assessment and, if needed, referral to a specialist.
